By law, according to the No Surprises Act H.R. 133 which went into effect on January 1, 2022, health care providers need to give clients or patients who do not have insurance or who are not using insurance an estimate of the bill for treatment services. This applies to patient who are paying out of pocket for services.
* This Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known at the time the estimate was created.
* You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes (under the law/when applicable) related costs like medical tests, prescription drugs, equipment, and hospital fees.
* The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. If this happens, federal law allows you to dispute (appeal) the bill.
* If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.
During Your Visit
-
Please bring your photo ID, insurance card, and any relevant medical records. Whenever possible, we recommend sending your records to our office in advance so our providers can review them before your visit. Relevant information may include recent lab results, imaging reports (e.g., X-rays, MRIs, DXA scans), actual image files, progress notes from other providers, and a list of current medications.
Athletes may also find it helpful to share training logs, performance data, or nutrition notes. If you’re unable to send your records ahead of time, you’re welcome to bring them with you to the appointment. -
Appointment times vary based on your reason for visiting and the complexity of the appointment. If imaging or lab work is needed, that can add to the overall length of your visit. Most appointments last between 30 and 90 minutes.
-
Medical: Your first visit includes a detailed discussion of your medical history, current symptoms, and athletic goals. Depending on your needs, the provider may recommend on-site imaging or lab work during the visit. You'll leave with a clear plan for next steps, which may include follow-up care, treatment options, or referrals to other specialists on our team.
Nutrition: At your first visit, you’ll get to know the registered dietitian and talk about your health history, eating habits, lifestyle, and goals. Together, you’ll explore ways to support your nutrition and well-being and create a personalized plan to get started. You'll also have time to ask questions and set realistic, meaningful goals.
